Intake Session Request
To schedule an intake, please fill out the form below. This will be a 45-50 min initial session in which I will ask more detailed questions about current symptoms, as well as gather important background information. Usually, after this session, I am able to share thoughts about diagnosis, treatment approach, and any additional assessment that may be needed. I also like to leave time during this session for you to ask any questions you may have about treatment.
